The principal institutional failure observed is not a lack of intent but a lack of pre-positioned authority: message architecture that maps legal constraints to stakeholder accountabilities and that assigns a single point of communicative authority for high consequence moments. That absence produces mixed signals, regulatory missteps and avoidable loss of institutional trust.
